Important Reminder: CAPF 34s Coming Due

Dear Chaplains and CDIs,

I know this is a very busy time for most of us, especially those who are involved in full time ministry, but I would like to respectfully request that you take a little time as soon as possible to complete your CAPF 34 for the second half of 2010.  It shouldn't take more than 15 or 20 minutes for most of us to complete the form.  If you have questions, please don't hesitate to ask.  The regulation establishing the requirement is CAPR 265-1, Section B, Para. 8.

I've attached PDF and MS Excel copies for your convenience.  Alternately, forms may be downloaded from the Wing Chaplain's Blog, or from NHQ.  Those of you who may not have access to Microsoft Office products may want to try Sun Microsystems' MS Office compatible freeware OpenOffice Suite.

Ch. Walters has asked that I collect the forms on his behalf this year, and compile the data for submission to the Region Chaplain.  It takes a little while to compile and double check the data, so I would appreciate receiving your reports as soon as you are able to complete them, but hopefully before Christmas.

If you complete the form electronically, you can email it to chaplain.pawg@gmail.com.  Paper forms may be mailed to PO Box 911, Warren, PA 16365-0911.  Please don't forget to submit a copy to your immediate commander and keep a copy for your personal record.
